He also denied that shrapnel caused her death.

Instead, Cheema said Bhutto was killed when she tried to duck back into the vehicle, and the shock waves from the blast knocked her head into a lever attached to the sunroof, fracturing her skull.

Who’s responsible? Allegedly a group with “ties” to Al Qaeda. Pakistan’s government apparently came to that conclusion from two findings: A) Reconstructing the attacker’s mangled head, B) Intercepting a “good job” memo from Baitullah Mehsud, a Taliban chieftain.
Update: Allah also points out that we’ll never know for sure exactly how she died. Per her husband’s request, no autopsy was performed.
